Worker Forms

All the relevant forms for members to access Incolink benefits and services are available online. Just click on the one that you want and download, print out, fill in and then send or fax to Incolink.

Incolink Initial Claim Form - You can apply for the Initial Benefit when you first become unemployed.
Incolink Balance of Funds Redundancy Benefits Claim Form - You can apply to withdraw the balance of money left in your account after your initial benefit has been paid.
Incolink Additional Payment Claim Form - This form is to be used when you have recently made a redundancy benefits claim to Incolink and contributions relating to the period prior to your termination are received at Incolink after the claim has been paid.
Incolink Beneficiary Notification Form - This form is to be used when nominating or changing nominated beneficiaries of your Incolink funds.
Incolink Registration for Free Employment Service Form - Employment Registration Form is to provide relevant information to any prospective employer seeking a worker for which your qualifications, skills and experience seem suited.
Incolink Apprentice Credits Application Form - This form is to be completed by the employer where the training agreement of an apprentice is cancelled/terminated or where the employment of a worker being a former apprentice
Incolink Prostate Cancer Screening Claim Form - Incolink will reimburse the gap between the cost of the doctor’s appointments and the tests involved in prostate cancer screening.
Incolink Breast Cancer Claim Form - Incolink will reimburse the gap between the cost of the doctor’s appointments and the tests involved in breast cancer screening
Incolink Transfer To Reciprocating Fund - this for is to be used if you wish to transfer your redundancy fund to an reciprocating funds provider
Incolink Genuine Redundancy Account Application Form - to apply to have your account transferred into an Incolink Genuine Redundancy Account (GRA)
Incolink Genuine Redundancy Account Cancellation Form - If you wish to cancel your application to account transferred into an Incolink Genuine Redundancy Account (GRA) - within 14 days of your application.
Incolink Genuine Redundancy Account Claim Form - If you hold an Incolink GRA and have been made genuinely redundant please use this form to claim your funds.
Incolink Genuine Redundancy Account Additional Funds Claim Form - This form is to be used when you have recently made a GRA claim to Incolink and there have been additional contributions made to Incolink after the claim has been paid.
